Almost half of the members of the American Association of Endodontics that participated in the following web based survey are using an adjunct, such as ultrasonic activation to aid in their irrigation technique. This new video deals with irrigation techniques and adjuncts that could improve irrigation efficacy. Notice the irrigant entering the isthmus of the messial root during ultrasonic activation. I hope you will enjoy this new video.

External invasive cervical resorption management video. 6% NaOCl was pressed against the resorption defect. The defect was restored using composite dual cured resin. The sutures were removed 10 days after the surgery because of patient related reasons. Ten days after surgery the healing seems optimal, the vitality tests are possitive and the patient is asymptomatic. I didn’t used Trichloro-acetic acid for coagulation necrosis of the resorptive tissue because of it’s high toxicity. I hope you will enjoy … [Read the rest]

The maxillary premolars have a higly variable anatomy. The maxillary premolars with three distinct roots are extremely rare, especially the second maxillary premolar. This new video describes the diagnosis and clinical management of first and second maxillary premolars with three distinct roots, drawing perticular attention to radiographic interpretation and access refinements. Enjoy!

Hello Buddies.
This female patient 30y wants here crowns to be renewed.
I planned so far root canal retreatment at 11/21/22 and perhaps on 12 also with post and build-up. Longterm provisoric crowns to wait for some healing. And afterwards zirconium crowns.
Some collegues made me aware of the occlusion and of the necessity to raise the occlusion up.
So what would you do?

I have just received a single tooth implant case back from the lab and thought I would share a simple technique to create a jig to help orient the abutment in the mouth and ensure it is fully seated. When implants have hex or double hex internal connections, incorrect orientation of the abutment can easily occur, in the mouth. Soft tissue can also sometimes interfere with abutment seating and, when using an implant system with a conical … [Read the rest]

Cleaning and shaping c-shaped canals could be really challenging. Walking your Ultrasonic file through the c-shaped canal configuration might be the answer to the effective instrumentation of such cases. Enjoy this new video.

The hard rock of endodontics stands for obliteration in Endodontics. Totally or partially obliterated canals present a constant challenge in clinical endodontics. Negotiating obliterated canals could be from difficult to nearly impossible. The risk of perforating is high as well as the risk of instrument separation. However, under the magnification provided by a dental operating microscope the whole procedure of dealing with obliterated canals could become very predictable. Complete root canal space obliteration associated with apical periodontitis … [Read the rest]

Hello Buddies,
I don’t know how it is in other countries but here when you treat a close relative (mom, dad, sister etc.) they don’t listen to you as a doctor but they treat you like their little kid! So I made two NSRCT of teeth 31 and 32 to my mother. The root canal treatment could have been different if she wasn’t afraid soo much from dentists. Sha had pulpal pain for several months but she … [Read the rest]

Hello Buddies!
Here is a border case between extraction and trying to safe these teeth. The periodontal diagnose is aggressive periodontitis. A GBR was tried before half year in some clinic. Soon after the graft and membrane procedure pulpal pain from 26 and 27 starts to torture the patient. He came in the office from the periodontologist with suppurating pocket between 26 and 27 and a sinus tract. Just a slight mobility was noticed that do not … [Read the rest]

Hello Buddies,
This is a case I did recently. The case itself was easy. However I added my complete protocol and also the complete diagnosis. Most of the time, referring dentists sent me very brief referral letters. I wanted to show that it is important to be complete in a diagnosis and what you have to think off, even if it is an easy case.
Kind regards,
Rafaël

Posting a small response takes less time than editing a video
That said, I told that I was going to post something this weekend, but I’ll have to postpone it till wednesday. But as an “amuse-gueule” here is a case I did this month.
Sometimes when the caries extends too deep, I still prefer an amalgam core above a composite core. This is not a definitve restoration, it will act as a core for a crown.

repare your cases for sharing. I thought about … [Read the rest]

The greatest fear about instrumentation in endodontics is file separation. New file designs, new alloys, different file movements they all aim to the elimination of file breakage inside the root canals. So much effort for the development of new instrumentation technology that won’t leave broken fragments behind and so little effort on the development of effective predictable methods of removing the broken fragments. Just a look in Marco Versiani’s micro-ct images of the root canal anatomy project … [Read the rest]

Third Maxillary Molar anatomy can be really challenging. The existence of MB2 canal in 3rd maxillary molars is a nightmare, especially when the mesial canals are highly curved. This new short video is about difficult anatomy management. 5 hours and about 30 files damaged is what it takes for such extreme cases. Maybe i should go implants
